2.   The nurse is caring for a client who has had extensive surgery and is in critical condition. The nurse notes that the complete blood count shows an 8 g/dl hemoglobin and a 30% hematocrit. Dextrose 5% in half-normal saline solution is infusing through a triple lumen central catheter at 125 mL/hr. The physician's orders implemented by the nurse include:

• Gentamicin (Garamycin) 80 mg I.V. piggyback in 50 ml D5W over 30 minutes.

• Ranitidine (Zantac) 50 mg I.V. in 50 ml D5W piggyback over 30 minutes.

• 250 ml of packed RBCs over 3 hours.

• Flush the nasogastic tube with 30 ml normal saline every 2 hours
